An SR22 is a form that verifies to the state that you have auto insurance with the state minimum liability limits. An SR22 form is requested by the state for certain drivers to provide proof of financial responsibility (i.e., liability insurance).

If you are required to maintain this form, you will be notified by the state.

Reasons that an SR22 form might be required:
